Overview
Medication Renewal Center is part of the dynamic centralized services offered within The Patient Contact Center. The MRC CMA/RMA supports Carle Health Primary Care patient's medication management, adherence, and refill needs by providing advanced level patient care and support. MRC CMA/RMA's utilize Physician led guidelines and protocols to assist in efficient decision making and enhances patient compliance with health maintenance and chronic medication management. The ideal candidate will enhance our talented team by offering strong communication skills, ability to critically think, and thrive in a fast-paced environment where you can make a difference in the lives of our patients.
This position has work from home opportunities after a minimum of 6 months of in-person training in Champaign, Illinois.
Responsibilities
- Collaborate with Medication Renewal Center nursing and clerical teams to maximize productivity and efficiency.
- Complete medication refill requests, care gap notifications, and order entry using established protocols.
- Review and process medication renewal requests received electronically, by fax, or by telephone.
- Maintain accurate and thorough EMR documentation related to medication renewals, patient care needs, and follow-up activities.
- Receive and enter faxed refill requests, pharmacy communications, and medication orders into the EMR.
- Order and schedule laboratory work and patient appointments in accordance with protocols.
- Review patient medication compliance through detailed medical record analysis.
- Communicate medication orders to pharmacies through verbal and electronic methods.
- Provide patient health and wellness education using approved resources and provider-directed materials.
- Ensure documentation is complete, accurate, and compliant with departmental standards.
- Adhere to medication management, refill guidelines, and established clinical protocols.
- Coordinate with primary care providers, nurses, and care teams to support patient needs.
- Identify care gaps and promote adherence to chronic medication management plans.
- Review current, past, and future appointments and laboratory results to support safe medication administration.
- Communicate with pharmacies regarding medication discrepancies, clarification requests, and refill concerns.
- Contact patients regarding required laboratory work, office visits, and follow-up services needed for medication refills.
- Communicate medication changes to patients, providers, care teams, and pharmacies to reduce medication errors.
- Disseminate provider-approved patient education and instructions.
- Schedule patient appointments and verify insurance coverage.
- Process patient concerns using symptom-guided protocols and departmental procedures.
- Accurately collect, document, and report patient information within the EMR.
- Maintain compliance with organizational policies, procedures, and quality standards.
- Assist with onboarding, training, mentoring, and orientation of new team members.
- Provide float support to Patient Contact Center Clinical teams, including Patient Advisory Nurse (PAN) and Patient Clinical Services (PCS), as needed.
- Support medication management and patient care coordination in a non-face-to-face patient care environment.
- Manage high-volume medication refill workflows and pharmacy communications.
- Monitor patient compliance with prescribed medications and recommended care plans.
- Facilitate communication between patients, providers, pharmacies, and clinical staff to ensure continuity of care.
