Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Responsibilities/Day in the Life:
- Receive new patient referrals from clients
- Create charts for all new patients
- Input medical information in new chart
- Help Care Coordinators with patient needs as applicable
- Foster relationships with their team to ensure the CareHarmony culture is positive and supportive.

Required Skills/Abilities:
- Great attention to detail
- Excellent organizational skills
- Excellent verbal and communication skills
- Excellent professional presence when dealing with colleagues
- Excellent time management skills with a proven ability to meet critical deadlines.
- Fundamental Skills with Microsoft Office Suite
Required Education and Experience:
- Medical Assistant certificate must be active and in good standing
Physical Requirements
- This position is sedentary and will require sitting for long periods of time
- This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time
- The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
